Shared Decision Making is a communication process in which a case manager and a client collaborate to make optimal health care decisions based on what matters most to the client. Because communication is a core competency for case manager to master, undertaking fluency in Shared Decision Making is an essential step toward delivery of personcentric care. The concept and principles of Shared Decision Making are presented as an implication to transitions of care.
At the conclusion of this activity, attendees should be able to:
Assess documentation and utilization best practices in preventing denials and appeals
Define Shared Decision Making
Identify process phases of SHARE
Incorporate Shared Decision Making elements into care transition conversations
