Addressing the issues, gaps, and barriers with transitions of care is a challenge for every population. There are specific issues in working with pediatric patients magnified for the complex and catastrophic pediatric patients and families. The focus of this course is to identify the positive solutions to extraordinarily complex problems for the pediatric population. Ms. Mullahy will address the trends in pediatric care and focus on management strategies to improve care coordination and the case manager's role.
At the conclusion of this activity, attendees should be able to:
Gain a realistic and empowering vision of their role in the pediatric health care delivery process
Learn to better appreciate and communicate their capabilities and contributions
Identify trends and opportunities for pediatric case management of transitions of care

Catherine M. Mullahy, RN, BS, CRRN, CCM, is the President and founder of Mullahy & Associates, LLC. She is widely recognized as one of the healthcare industry's foremost leaders in advancing case management standards and practices.
