The importance of safe care transitions is an essential aspect of health care quality and patient safety. Although focus on transitions of care (TOC) exploded in the first decade of the 21st century, it is not a new concept. The failures of care transitions have very real financial consequences to acute care settings. With the creation of integrated health care networks and risk-sharing agreements, financial penalties extending across the entire care continuum are on the horizon.

At the conclusion of this activity, attendees should be able to:
Review the historical development of TOC
Identify essentials of safe and effective TOC
Define the various transitions of care models
Explore the rise in importance of community-based initiatives
Discuss quality and outcomes associated with TOC
This standalone course is also included as a portion of the Core Body of Knowledge (CBK) online course. They may be taken individually to earn incremental CME/CE credits, recertification credit, or towards the Transitions of Care sub-specialty designation. However, HCQM Exam candidates taking the CBK course would not benefit from the similar material nor be able to claim credit for both courses for initial certification.

This course is intended for Physicians of all specialties, Nurses, and Other Health Care Professionals with appropriate CME / CE credit offered for each profession.

All participants must achieve a final assessment score of at least seventy percent (70%) for recertification and continuing education credits. You have 60 days to complete the course after ordering.

Teresa M. Treiger, RN-BC, MA, CCM, CHCQM, FABQAURP
Mrs. Treiger served as on the Board of Directors for the National Transitions of Care Coalition and is a Past National President for the Case Management Society of America. As Principal of Ascent Care Management, Teresa provides professional education and consultation services to accreditation organizations, health care organizations across the healthcare continuum as well as private case management services. She is the co-editor and chapter author of CMSA Core Curriculum for Case Management (3rd ed.) with Hussein Tahan and co-author of COLLABORATE for Professional Case Management: A Universal Competency-Based Paradigm with Ellen Fink-Samnick.

Cheri Lattimer, RN, BSN
Cheri Lattimer is Executive Director for the National Transitions of Care Coalition (NTOCC) and CEO of Integrity Consulting & Advocacy. Her leadership in quality improvement, case management, care coordination, and transitions of care is known on the national and international landscape. Her work has been featured in several publications, including the Healthcare Executive, Dorland’s Case In Point, Professional Case Management, and Seniority. She is affiliated with various professional organizations and maintains active roles on several national boards and committees including URAC’s Redefined Health, Roundtable on Critical Care Policy, CMS Caregiver Workgroup, ACHIEVE, and the American Nurses Association.
