Working With the TOC Pharmacists: Beyond Medication Rec

Pharmacists, as members of the interdisciplinary care team, have a crucial role and impact in improving transitions of care (TOC) for patients, their identified family caregivers, and providers within the health care continuum. What we find today is their role is not always acknowledged within the collaborative care team and we often exclude an important discipline that can support not only medication management services but interventions toward improving transitions and care coordination at all levels of care/service in our health care system. This course addresses the gaps and barriers that hinder TOC pharmacists and offer principles and examples of innovative collaboration that support pharmacists as an integral element of quality care transitions between patients, their identified family caregivers, and providers.
At the conclusion of this activity, attendees should be able to:
Identify the gaps and barriers in transitions of care (TOC) and medication management services
Discuss the role and function of TOC pharmacists and their potential impact to improve care coordination
Analyze the Transitions of Care Pharmacists 10 Principles and how to incorporate this process into daily practice at the acute and post-acute levels of care

This course is intended for Physicians of all specialties, Nurses, and Other Health Care Professionals with appropriate CME / CE credit offered for each profession.

All participants must achieve a final assessment score of at least seventy percent (70%) for recertification and continuing education credits. You have 60 days to complete the course after ordering.

About the Author

Sara Panella, PharmD, BCPS
Manager of Pharmacy Ambulatory Care, Population Health and Transitions of Care - Baptist Health South Florida

Sara M. Panella, PharmD, BCPS, is currently the Manager of Transitions of Care (TOC) Pharmacy Services for Baptist Health South Florida Pharmacy Enterprise, in Miami, Florida. Dr. Panella obtained her Doctor of Pharmacy from the Ernest Mario School of Pharmacy (EMSOP), Rutgers, The State University of New Jersey in 2008. After graduation, she completed a Pharmacy Practice Residency at Yale-New Haven Hospital followed by a PGY-2 Specialty Pharmacy Residency in Drug Information at Robert Wood Johnson University Hospital/EMSOP.

She started her career in TOC at University of Maryland Medical Center in Baltimore, MD and has since developed TOC programs there as well as at Memorial Regional Hospital in Hollywood, FL. Dr. Panella has also held academic positions at EMSOP as a Clinical Assistant Professor and at Nova Southeastern University as an Assistant Professor, and has also been a previous Residency Program Director for a PGY-2 in Transitions of Care. Dr. Panella holds a Lean Green Belt Certification. Dr. Panella is currently working with the National Transitions of Care Coalition (NTOCC) to develop a position on the role of pharmacists in Transitions of Care.
