Optimizing Coordination Among Mid-Revenue Cycle Stakeholders

Too often the revenue cycle world sees poor communication between hospital departments, which leads to incalculable inefficiencies and delays in patient care. Although Tampa General Hospital is one of the leading healthcare systems in Florida and the country, it fell victim to this trap. Tampa General and Brundage Group worked collaboratively across multiple departments to re-invigorate lines of communication between the C-suite, Case Management, Utilization Management, and Emergency Room utilizing a GE-powered "Care Command" center that assisted with bed control, throughout, expediting radiology studies, and addressing discharge barriers. Together the team engaged the problem by finding several programs (ED throughout, CM discharge barriers, UR support) that encompassed multiple departments. Of course, there were multiple challenges along the way, including a foundation of inter-departmental strain, hospital politics, and understaffing issues. The team navigated these challenges by clearly defining roles and responsibilities while at the same time encouraging team members to engage their counterparts in sister departments. Outcomes included improving LOS, decreasing "avoidable days," and facilitating ED discharge of unnecessary admissions, thereby increasing open beds on the floors.

Explain the importance of a centrally located "care communication" center to oversee and guide decisions impacting patient throughput during the episode of care.

Dr. Timothy Brundage is Chief Executive Officer and Physician Advisor at Brundage Group.
He is a past board member of the Association of Clinical Documentation Improvement Specialists (ACDIS) and is co-chair of the CDI committee for the American College of Physician Advisors (ACPA).
Dr. Brundage travels the country educating physicians, with a focus on breaking down departmental silos in hospital organizations by unifying clinical documentation across quality, utilization, denials and CDI. He is passionate about increasing the value of the physician advisor role within hospital organizations.

Dr. Brundage is a frequent presenter at the ACDIS annual conference and its state chapter meetings, as well as ABQAURP’s Annual Conference. He is a speaker for AHIMA at its annual meeting and its CDI Summit, as well as for ACMA state chapter meetings and the national ACMA Leadership and Physician Advisor Conference. Dr. Brundage is a presenter at the RAC Summit National Physician Advisor and Utilization Review Boot Camp.

Manager, Care Coordination - Tampa General Hospital

Tara Sahno, MSN, BSN, ACM-RN is a highly skilled, nationally certified Case Manager with 16 years of Emergency Department nursing experience. She holds a Master’s Degree with fields of study in Public Health and Administration at University of Hartford, CT and a Bachelor’s Degree in Science, Nursing at Saint Joseph College, CT. Ms. Sahno’s current professional role is as Manager of Care Coordination at Tampa General Hospital in Tampa, FL.

Ms. Sahno’s current professional initiatives are to increase efficient processes in the delivery of Case Management efforts; collaborating with clinical teams and providers to deliver safe efficient care through appropriate testing and clinical interventions based on patient signs, symptoms, and diagnosis; and establishing new processes in collaboration with procedural teams to increase scheduling efficiency through identification of appropriate hospital utilization and clinical progression vs. discharge to return as outpatient. Tara has developed and presented, in partnership with a colleague, professional CEUs on Caregiver Fatigue. She also completed a data analysis of current productivity and process improvement initiatives with the Tampa General Hospital Nurse Executive Team.
