The U.S. health care system continues to shift to value-based care, defining quality through measures in various care settings. Pharmacists and pharmacies involved in transitions of care can help ensure safe and appropriate medication use throughout the patient's journey. This course will highlight the state of quality medication use as it relates to transitions of care.
At the conclusion of this activity, attendees should be able to:
Describe the shift from volume to value and quality in the U.S. health care system
Identify the value equation in health care
Highlight relevant medication use quality measures that appropriate transitions of care can impact

This course is intended for Physicians of all specialties, Nurses, and Other Health Care Professionals with appropriate CME / CE credit offered for each profession.

All participants must achieve a final assessment score of at least seventy percent (70%) for recertification and continuing education credits. You have 60 days to complete the course after ordering.

About The Authors

Chris Kotschevar, PharmD, RPh, CPHQ
Christopher Kotschevar is the Director of Stakeholder Engagement for the Pharmacy Quality Alliance. Dr. Kotschevar develops and maintains relationships with PQA’s diverse stakeholders, seeking to connect members with opportunities to work together to improve quality medication use. He also supports PQA’s public affairs strategy to broaden awareness of PQA’s expertise in quality medication use.
Prior to this role, Dr. Kotschevar completed a post-graduate Executive Fellowship with the Pharmacy Quality Alliance, focusing his work on growing PQA’s educational offerings in medication use quality and leading research examining barriers and facilitators of specialty pharmacy prescription turnaround time.
Dr. Kotschevar earned his Doctorate of Pharmacy from the South Dakota State University College of Pharmacy and Allied Health Profession. Upon graduation, he was awarded the college’s highest honor, Distinguished Graduate, for his professional service and scholarly work throughout his time at SDSU.
