The role of case managers, social workers, pharmacists, physicians, and other health care professionals in helping patients navigate the health care continuum is crucial to achieving improved health outcomes. National health care policy and practice trends toward integrated care models that address the social determinants of health are redefining the way we deliver health care. This course will consider current policy and practice trends supporting efforts to eliminate health care disparities and will identify key strategies and practical action steps health care professionals can take, including increased engagement in practice research and knowledge dissemination activities for improved integration of care across practice settings.
At the conclusion of this activity, attendees should be able to:
Identify key health policy initiatives and related current practice trends in health care delivery to address the social determinants of health in pursuit of health equity.
Review strategies that health care organizations and clinicians can implement to address health-related social needs to promote more equitable healthcare for their patients.
Develop plans to engage in or promote research and knowledge dissemination activities that produce innovative health equity practice interventions across the continuum of care.

This course is intended for Physicians of all specialties, Nurses, and Other Health Care Professionals with appropriate CME / CE credit offered for each profession.

All participants must achieve a final assessment score of at least seventy percent (70%) for recertification and continuing education credits. You have 60 days to complete the course after ordering.

About The Authors

Raine Arndt-Couch, DSW-C, JD, LCSW, CCM
Raine Arndt-Couch, JD, MSW, LCSW, CCM has over 15 years of experience in clinical social work, medical case management, national health care policy, and leadership roles across health care practice settings and with diverse populations. She currently serves on the CMSA National Board of Directors and as Senior Manager of Social Services for Blue Shield of California Promise Health Plan operating in Los Angeles and San Diego counties. In her role, Raine provides leadership and operational oversight of CalAIM Community Supports programming and Behavioral Health Care Management services for Medi-Cal beneficiaries. Raine is a proud social worker who is passionate about the integration of medical, behavioral, and social care in health care delivery as a means to achieve health equity. She earned both her master’s in social work (MSW) and Juris Doctorate (JD) from the University of Hawaii at Manoa and is a Doctorate in Social Work (DSW) Candidate at the University of Southern California.
