During transitions of care (TOC), medication errors are commonly attributed to inadequate medication reconciliation practices. The risk of medication errors can be compounded in patients with multiple comorbid diseases, or those with complex pharmacotherapeutic regimens, such as those patients suffering from pain conditions. Persons undergoing TOC are susceptible to interruptions in their pain management regimens which can result in reduced quality of life, increased risk of adverse events, higher likelihood of hospital readmission, and increased healthcare spending. This presentation will review best practices in verifying, clarifying, and reconciling analgesic regimens in persons with acute and/or chronic pain to ensure safe and effective management during TOC.
At the conclusion of this activity, members of the interprofessional team should be able to better:
- List steps to verify a medication regimen with a patient suffering from acute and/or chronic pain conditions
- Describe clinical considerations in clarifying the appropriateness of an analgesic regimen
- Identify examples of medication discrepancies that can occur when reconciling an analgesic medication regimen
