This 8-hour online video course explores today's complex health care delivery as it continues to evolve into a high-quality, value-based care system; improving patient outcomes, reducing costs, and increasing value for all stakeholders. As regulatory changes unfold, the interprofessional health care team continues to seek strategies to coordinate patient needs, reduce burdens on providers, and ensure reimbursement is maximized without fear of penalty. Communication between providers and throughout care settings is integral in meeting these needs. ACCREDITATION & CREDIT
In support of improving patient care, the American Board of Quality Assurance and Utilization Review Physicians (ABQAURP) is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

This activity was planned by and for the healthcare team, and learners will receive 8 Interprofessional Continuing Education (IPCE) credit for learning and change.

Physicians

ABQAURP designates this enduring material for a maximum of 8.0 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

About the Authors:

Karen Clark, MD, MACP, FRCP, CHCQM‑PHYADV, FABQAURP

Dr. Clark provides Physician Advisory Services to the West Virginia University Health System and serves as Medical Director of Utilization Management and Physician Advisory Services. She is also a Professor of Medicine in the Department of Internal Medicine at WVU Medicine. Dr. Clark holds Mastership in the American College of Physicians and Fellowship in the Royal College of Physicians (London), and her funded research has supported health initiatives, national presentations, and curriculum development. She is the recipient of the CHCQM Diplomate Achievement Award for a sustainable, high‑impact quality improvement initiative. Dr. Clark also led WVUH’s Complex Care Plans for High Utilizers pilot, now an interdisciplinary program that has achieved significant improvements in safety, utilization, and resource stewardship.

Tabassum Salam, MD, CHCQM, MBA, FACP

Dr. Tabassum Salam is a board-certified Internal Medicine physician with 20+ years of multi-faceted experience in healthcare and medical education. Keeping direct patient care as a constant point of reference, she has held various leadership roles in health systems and national physician membership organizations. Dr. Salam has also held health system-wide leadership roles in population health, working with national clinical, administrative, and financial leaders and architects of value-based payment models. As VP of Patient Experience at a large health system, Dr. Salam also liaised closely with executive, physician, nursing and Human Resources leadership on a data-driven approach to improving patient experience and HCAHPS scores.

Sohini Gupta

Sohini Gupta leads AHIP’s advocacy teams in Washington, D.C., and in all 50 states, engaging with legislators and policymakers on matters impacting coverage, care, and patient outcomes. Her 25 years of experience includes government affairs leadership roles at UnitedHealth Group; WellCare, which was acquired by Centene; Global Medical Response; the American Hospital Association; Baxter Healthcare; and Mehlman Castagnetti Rosen & Thomas. 

Gerard R. Dache

Mr. Gerard Dache has over 40 years of program management, public speaking, and leadership experience in healthcare, blockchain, information technology, and process improvement. He is the Founder, Executive Director, and a Member of the Board for the Government Blockchain Association (GBA). In this role, Mr. Dache created over 50 specialized working groups, including the Healthcare Working Group, focused on using blockchain to drive better health outcomes and more efficient healthcare administration.

Linda V. DeCherrie, MD

Dr. DeCherrie’s professional passion is home based medical care. She is the Vice President, Clinical Strategy and Implementation at Medically Home which is a technology-enabled services company that provides all the necessary tools to allow medical providers to provide hospital at home. Previously she was the at Mount Sinai Health System for 18 years and most recently was the Clinical Director of Mount Sinai at Home, the home-based service line. Dr. DeCherrie is a Clinical Professor in the Brookdale Department of Geriatrics and Palliative Medicine at the Icahn School of Medicine at Mount Sinai.

Mark Michalski, MD

Mark Michalski, MD, is CEO of Ascertain, a healthcare AI startup focused on providing healthcare enterprises with best-in-class automation. He has led several successful startups previously, including Butterfly Network (NYSE: BFLY) and Hyperfine Research (NASDAQ:HYPR). Dr. Michalski has also held roles in tech companies such as Amazon and Google—most recently serving as head of strategic business development for HCLS at Amazon, where he led Amazon-wide HCLS partnerships and initiatives, and as World Wide Leader of HCLS AI/ML BD at AWS.

Michiel Peters

Michiel Peters leads the advocacy initiatives at the Global Coalition on Aging (GCOA) and is a Senior Director at High Lantern Group, a strategic consulting firm that helps position organizations, their leaders, and their ideas in the public arena. He is an expert communications professional and strategist with over a decade of experience in the public and private sectors. Michiel worked on local, state, and national campaigns before serving as a senior press officer in the Dutch Parliament. After eight years of working in the public sector, he transferred to the private sector to build up the corporate and public affairs functions for a large family-owned multinational in the agricultural industry.

Traci Fisher, NBC-HWC, PCC ICF

Traci is an Executive Wellness & Leadership Coach who helps executive leaders unlock their peak cognitive energy, prevent burnout, and excel in personal & professional leadership. With three decades of experience and a blend of compassionate support and straight-talk, she helps professionals not just survive but thrive. A West Point graduate, former military officer, and helicopter pilot, Traci brings battle-tested insights on high-stakes decision-making, adaptability, and resilience — essential skills for today's healthcare environments.

Elizabeth A. Cook, PharmD, BCACP, CDCES

Dr. Elizabeth Cook is a Clinical Pharmacist Practitioner at the Robert J. Dole VA Medical Center in Wichita, Kansas. She is a board-certified ambulatory care pharmacist, as well as a certified diabetes care and education specialist. In her current role, she operates under a collaborative practice agreement to facilitate chronic disease state management during pharmacist-led appointments as well as a polypharmacy expert for complex patients in her facility’s interdisciplinary geriatrics clinic. Beyond direct patient care, Dr. Cook supports pharmacoeconomic and population management quality improvement initiatives, as well as serves as the research coordinator for her facility’s PGY1 Pharmacy Residency Program.
