About the Authors:
Collin M. Clark, PharmD, BCPS, BCGP
Collin M. Clark is a Clinical Assistant Professor in the Department of Pharmacy Practice at the University at Buffalo School of Pharmacy and Pharmaceutical Sciences (UB SPPS). He completed his Doctor of Pharmacy at the UB SPPS in 2017. He then went on to complete a PGY-1 Pharmacy Practice Residency at the Veteran Affairs Western New York Healthcare System and a T32 Health Services Research Fellowship at the University at Buffalo Jacobs School of Medicine and Biomedical Sciences. He currently practices as a Clinical Pharmacist in Transitions of Care at Buffalo General Medical Center. His research centers on the implementation and assessment of pharmacist provided clinical services. He has a particular interest in studying the pharmacist’s role in transitional care management across the continuum of care and medication optimization strategies for older adults with complex medical conditions.
