NTOCC-Coming of Age: Social Determinants Matter

In an era driven by cost containment, demographic imperative, and uncertainty about public financing, social determinants of health have become the "new-old" focus of the field of aging. This increasing recognition of psychosocial and environmental factors as root causes of illness creates opportunities for interdisciplinary collaboration among practitioners, educators, and researchers in aging from the health care and social care sectors.
At the conclusion of this activity, attendees should be able to:
Demonstrate why and how social determinants and care coordination matter as people come of age
Describe initiatives and care models that influence and expand team membership and scope to better address the comprehensive health needs of older adults and their families
